Our approach is person centred and outcome based. When we first receive a new referral we ask for significant information about the young people that will help us understand the needs of the young person and how we can best meet their needs and wishes.
These preferences may include the location they wish to live, in relation to work/education family and friends and/or if they wish to share a flat/house. If it is identified that a placement is required, the young person will be given an opportunity to view a number of properties.
When culturally specific support is required, we arrange for interpreters to be involved and we will form links with appropriate community based groups to provide introductions. This encourages social integration with others who share the same cultural background, religion, and/or language.
Once the Pathway Plan is agreed with the young person and social worker, we’ll explore the most effective ways of working with the service user and begin to plan out the steps we’ll take together by creating an individual support plan.
